Removes the stale Specifications.md (it had drifted far from the code — dead --overview flag, old raw_url signature, old model sentinels, Python 3.11 refs).\n\nSalvaged the still-useful content into AGENTS.md under "Design notes": non-goals, sequential-fetch + rejected-dependency rationale, intent-file parsing rules, config invariants, sorting, monorepo detection, cache strategy.\n\nAlso fixed a stale --overview reference in AGENTS.md and pointed the old spec reference at the mkdocs site.
